If you are an existing client, a Loan Broker, Investor or Lender familiar with the information and documents necessary to set up a Trust Deed or Mortgage for Servicing, this form is for YOU.

By completing the following form, the Note Servicing Center has made it possible for you to enter the information to set up a new account directly into our proprietary system for set-up within 48 hours after receipt of the required information and documents.

Documents, for example: a copy of the Trust Deed Note or Mortgage, can be uploaded to our office at the end of this form or sent by:  email to [email protected], fax to 559-665-3457, or by mail to Note Servicing Center 3275 E Robertson Blvd., Ste. B, Chowchilla, CA 93610.

Please keep in mind in order to begin the servicing setup process the executed Note, Servicing Agreement and/or Assignment of the Note and Deed of Trust or Mortgage are required. Additional documents may be requested depending on the type of account you are sending to our office, an account representative may contact you if additional documents are needed.
